You have probably encountered situations where you needed to conduct evaluations related to your governance routine, whether it's a survey about company processes, a new project, or even assessing a board.
You can conduct these questionnaires using our Forms feature.
This is an exclusive feature of the Enterprise plan, and to help you understand how to use it, I'll provide some examples:
Example 1: To evaluate the company's strategic planning and policies.
Example 2: To gather insights about the future vision of financial performance and risks based on the members.
Example 3: To collect ideas to assist in monitoring the company's performance.
Now, let's see how you can create and respond to forms in practice:
How to Create a Form:

Click on "Forms" in the bottom left corner, then select the "Create New" button.

Choose the board to which the survey will be associated and click "Next".

Enter the title of the survey and select your respondents.

Set the deadline for completing the form. You can also choose whether the survey will be anonymous or not.

Create the first section (first topic) and choose the type of response that best fits your form. Atlas currently offers three formats:
Free answer: Where respondents provide answers in their own words.
Multiple choice: Respondents select answers from a predefined list.
Rating: The response is used to assess the level of the question, such as "good" to "bad," "excellent" to "could improve," or a scale from 1 to 5.

After finalizing the survey setup by selecting the number of sections and types of responses needed, click on the three dots in the upper right corner and select "Send."
Confirm the sending to notify all respondents by email about the pending survey.

How to Respond to a Form:
- You can access the form by clicking on your inbox and filtering by "Forms" to quickly find the one you need to respond to.
- Another way is to access it directly through "Forms" and view the second section, which lists all the surveys. If the title is blue, it means it hasn't been responded to yet, and the section will indicate "no" if the pending survey hasn't received your response.
- Once you locate the survey, make your selections and click "Submit Response".
